I've watched this scene many times - fake or real, it's still one of the hottest I've ever seen in a mainstream film. The unrated version that everyone looks at has obvious exposure manipulation happening during "those scenes" - the term "dodging and burning" is used by photographers - meaning the exposure of the film has been messed about so that Rourke's genital area is completely dark. So we don't know what's happening down there. That does suggest they're trying to mask something - maybe the fact NOTHING is going on down there. But my second piece of "evidence" is the simple fact that I don't think Otis was a good/experienced enough actress to fake it as well as she does. Frankly, unless Zalman King someday releases the original raw, unfiltered footage, or a Blu-ray release reveals more, we'll probably never know for certain. Which makes the scene even hotter.
